Quit2Heal Mobile Application Privacy Statement
Quit2Heal (We or Us), the research team that created the Quit2Heal application for smoking cessation at the Fred Hutchinson Cancer Center in Seattle, Washington, takes your privacy seriously. We respectfully request that you review our Privacy Statement carefully so you understand our privacy practices. This Privacy Statement explains how We collect, use, disclose, and safeguard the information you provide to Us so you can make an informed decision when using Quit2Heal. This Privacy Statement covers both our website and mobile application (Service). By using Quit2Heal or otherwise providing your information to Us, you agree to the terms of this Privacy Statement. If you do not agree with the terms of this Privacy Statement, please stop using Quit2Heal.
INFORMATION WE COLLECT
We collect some non-personal information when you access Quit2Heal through the mobile application. Non-personal information includes information that cannot be used to identify you, individually, like anonymous usage data, number of clicks that you make as you navigate the website or mobile application, etc. The collection of this information is regulated by the Fred Hutchinson Cancer Center’s Human Subjects Internal Review Board.
HOW WE USE PERSONAL INFORMATION
We may ask you to provide Us with some information to create your User profile and to support your use of Quit2Heal. We describe how We use or disclose your information in more detail here:
User Information. Quit2Heal includes an engaging and personalized experience for quit smoking program participants (Users). To effectively deliver Quit2Heal to our Users We may collect, store, and use information that you provide to Us when you use Quit2Heal, such as the number of cigarettes you smoke. The use of this information is regulated by the Fred Hutchinson Cancer Center’s Human Subjects Internal Review Board.
Communications. If you email us at Quit2Heal@fredhutch.org with a question or request, We may use personally identifiable information you provided to Us in that we will respond to you using your email address to provide the information that you requested, or to update you about changes to our Privacy Policy if any occur. We will not collect or save this personally identifiable information.
Research. We use de-identified analytics information (such as number of downloads or levels viewed in the Quit2Heal app) to measure Quit2Heal’s impact and effectiveness to Users, and to continue our research at the Fred Hutchinson Cancer Center, which may result in published reports or articles. De-identified information is used in our reports, research, surveys and results. In order to protect your privacy, We do not collect any personally identifiable information, and will not include any in our research reports.
HOW WE PROTECT YOUR INFORMATION
When you share information with Us it is protected both online and offline. The computers and servers We use to store this information are kept in secure environments. Only employees of the Fred Hutchinson Cancer Center with the need to access your information for the research study are granted access to your information.
ACCESS AND CONTROL TO YOUR INFORMATION
Your Rights. We are required by law to: (1) protect the privacy of your Information, (2) provide you this notice explaining our responsibilities and privacy practices when We collect your Information, (3) notify you of a breach of unsecured Information, and (4) follow the terms of our Notice.
Data storage. We store de-identified information for 10 years, and the retention period may be extended at Our request if approved by Fred Hutch’s Institutional Review Board.

MISCELLANEOUS PROVISIONS
Devices. We may collect information from or about the computers, phones, or other devices where you install or access Quit2Heal, depending on the permissions you've granted. We may associate the information We collect from your different devices, which helps Us provide consistent use of Quit2Heal across your devices. For example, connection information such as the name of your mobile operator or ISP, browser type, language and time zone, and IP address.
Cookies. We may use cookies and similar technologies to provide support of Quit2Heal.
Legal Proceedings. We may disclose your Information in response to a court or administrative order, subpoena, discovery request, or other lawful processes.
UPDATES TO PRIVACY STATEMENT
We will notify you through our Website or email if provided of any significant changes made to our privacy practices by updating our Privacy Statement, and include the opportunity to stop using Quit2Heal.
